Information for Human Trafficking Victims
TVS is committed to working with victims of human trafficking and complies with the Consumer Financial Protection Bureau’s (CFPB) rule to assist those who are victims of human trafficking.
Instructions for submitting a request to block human trafficking information:
The Fair Credit Reporting Act (FCRA) requires the following information from any person requesting a block of information on their background check report:

Proof of Identity
- a. Provide your full name (first, middle, last, suffix)
- b. Any previously used names
- c. Current and/or recent full address (street number and name, apartment number, city, state, and zip code)
- d. Social Security Number
- e. Date of Birth
Additional proof of identity if needed. Copies of government-issued identification documents, utility bills, and/or other methods of your identity.

Trafficking Documentation, include a minimum of one of the following:
- a. A determination of victim status from a federal, state, or tribal government entity
- b. A determination of victim status from a non-governmental organization or member of a human trafficking task force authorized by the government to make such a determination
- c. A self-attestation that you are a victim of trafficking. The statement or an accompanying document must also be signed or certified by a government entity, a non-governmental organization or human trafficking task force, or a court.
